  • S – Supranuclear vertical gaze disorder
  • Y – Yes, it’s also known as dorsal midbrain syndrome
  • N – Nystagmus during attempted upgaze (convergence-retraction nystagmus)
  • D – Downward ocular deviation (the “setting sun” sign)
  • R – Retraction of the eyelids (Collier’s sign)
  • O – Ocular features include skew deviation and pseudoabducens palsy
  • M – Midbrain tumors, cysticercosis, and stroke can cause it
  • E – Evident in hydrocephalus from aqueductal stenosis
